WELCOME TO FBRA

 

 

 

 Thank you for giving us the opportunity to be your healthcare provider! We have compiled some helpful information to ensure your experience with us is efficient and stress-free. 

 
Before your appointment:
  • Please have your insurance card ready when calling to make your first appointment.

  • We accept most major insurances and PPO plans. There is limited availability for new Medicare patients. We are not Medicaid providers.

  • When calling, request an invitation to our patient portal so you can review your contact information, fill out a medical history form, and view upcoming appointments with our office!

  • To expedite your diagnosis, please have medical records mailed or faxed to our office prior to your appointment. If you have a copy of your medical records on hand, we welcome you to bring them to your initial visit. Relevant records include: exam notes from your referring physician, recent laboratory reports, and imaging reports.

  • Expect a reminder call or text two days before your appointment, we will call the number(s) you provided when you scheduled your visit.

  • If you need to cancel or reschedule your appointment, please call our office at least 24 hours in advance. Due to high patient demand and the limited availability of appointments, effective January 1, 2020 FBRA has instituted a "No Show and Late Cancellation Fee" policy that will result in a $50 fee if you cancel your appointment with less than 24 hours’ notice or do not show up for your appointment. Patients who cancel their appointments at least 24 hours in advance will not be charged a fee. Timely cancellation will allow this appointment time to be offered to another patient.

  • If a referral is required by your insurance provider it is patient responsibility to contact your PCP prior to your appointment and present a valid referral before your office visit.   

  • Co-payments, co-insurance, and or deductibles are collected prior to office visits. Appointments will be rescheduled if co-payment is not paid.

Payment for services at Fort Bend Rheumatology Associates:                                                              
  • Discounts cannot be given on co-payments, co-insurance, or deductible due to our contractual agreements with the insurance companies.

  • If you have a policy that requires  a referral and or authorization such as HMO you are responsible to contact your PCP to obtain this prior to your office visit, failure to do so will result in a balance that will be responsible to the patient.

  •  Any insurance claims that have been denied will become the patient’s responsibility this includes failure to obtain a referral. Self pay (no insurance) fees are payable in full at the time of service.

  •  You will be required to pay a minimum of 50 % from any outstanding balances before being seen by a provider. Please review your explanation of benefits received from your insurance carrier, which will clarify any balance due from you.  

  • Cash, personal check, debit card, Visa, Mastercard, American Express, and Discover are accepted. Credit card payments may be made in person or by phone. Personal checks may NOT be post-dated. Any returned check from the bank for insufficient funds will result in the patient’s account being assessed a $25.00 fee per check returned. FBRA will not accept checks from patients who have previously had two returned checks.

 
What to expect at check-in:                                                                                                                 
  • Please arrive for your first appointment approximately 10 to 15 minutes early. To respect our doctors' and patients' time, we require patients who are 15 minutes late or more to their appointment to reschedule.
  • You must present photo ID and your insurance card(s) at check in.
  • To keep our fees to a minimum, we require payment for co-pays, deductible, or co-insurance at the time of service. Payment is due when you check in. After your visit has been processed by your insurance(s), if you believe you are due a refund for over payment, notify our office at 281-980-1742. Please allow a minimum of 15 business days for investigation and any refund processing. 
  • New patient forms are available for you online to expedite the process of check in.​​
