Before your appointment:
  • Please have your insurance card ready when calling to make your first appointment.

  • We accept most major insurances and PPO plans. There is limited availability for new Medicare patients. We are not Medicaid providers.

  • When calling, request an invitation to our patient portal so you can review your contact information, fill out a medical history form, and view upcoming appointments with our office!

  • To expedite your diagnosis, please have medical records mailed or faxed to our office prior to your appointment. If you have a copy of your medical records on hand, we welcome you to bring them to your initial visit. Relevant records include: exam notes from your referring physician, recent laboratory reports, and imaging reports.

  • Expect a reminder call or text two days before your appointment, we will call the number(s) you provided when you scheduled your visit.

  • If you need to cancel or reschedule your appointment, please call our office at least 24 hours in advance. Due to high patient demand and the limited availability of appointments, effective January 1, 2020 FBRA has instituted a "No Show and Late Cancellation Fee" policy that will result in a $50 fee if you cancel your appointment with less than 24 hours’ notice or do not show up for your appointment. Patients who cancel their appointments at least 24 hours in advance will not be charged a fee. Timely cancellation will allow this appointment time to be offered to another patient.

  • Payment for Services Fort Bend Rheumatology Associates will file charges for office services, and accept the contractually agreed upon amounts. The patient is responsible for the co-payment, co-insurance, and deductible, if any. Please remember these amounts are dictated by specific insurance plans. Discounts cannot be given on co-payments, co-insurance, or deductible due to our contractual agreements with the insurance companies. Co-payments are collected prior to office visits. Appointments will be rescheduled if co-payment is not paid. Any insurance claims that have been denied will become the patient’s responsibility. Self pay (no insurance) fees are payable in full at the time of service. You will be asked to pay a minimum of 50 % frany outstanding balances while visiting our office or at least 50% of your past due balance before being seen by a provider. Please review your explanation of benefits received from your insurance carrier, which will clarify any balance due from you.

What to expect at check-in:
  • Please arrive for your first appointment approximately 10 to 15 minutes early. To respect our doctors' and patients' time, we require patients who are 15 minutes late or more to their appointment to reschedule.

  • You must present photo ID and your insurance card(s) at check in.

  • To keep our fees to a minimum, we require payment for co-pays, deductible, or co-insurance at the time of service. Payment is due when you check in. After your visit has been processed by your insurance(s), if you believe you are due a refund for over payment, notify our office at 281-980-1742. Please allow a minimum of 15 business days for investigation and any refund processing.
